Terms & Eligibility

Be a Haitian citizen (proof required).
You must be present in person for fingerprinting and photo.
For minors: parental consent + presence of at least one parent.

Birth certificate or extract from the archives (original + copy).
National identity card (NID), if available.
2 recent passport photos (consular format) if requested.
